Parental Forms (School Year 2024-2025):

Please review each of the documents below (scroll all the way down to the bottom to view each one) with your child before you BOTH sign our Parental Release Form. The Parental Release Form and the Student Activity Form are the ONLY TWO FORMS YOU WILL NEED TO PRINT AND SIGN and have your child bring in on the first day of school (along with the activity and class fees), to give to their Advisor.
