    Please read these instructions before filling out the forms.

    Dear Parents and Athletes:

    There are two forms (InterScholastic Athletics Parent Consent Form and InterScholastic Sports Examination) that must be completed before any student can participate in try-outs, practices, or competitions for any extra-curricular athletics or sports team. Please follow the directions below to ensure that forms are completed properly. You can download the forms from this section of our website.

    InterScholastic Athletics Parental Consent Form

    At the top of the form:

    • Student's name
    • Date of birth 
    • Official class (2012, 2013, 2014, etc.)
    • OSIS # (which can be found on your student's school ID)

    At the bottom:

    • Emergency Contact numbers
    • Parent/guardian signatures
    • Date

    IMPORTANT CHANGE: If your student athlete is a transfer student, please be sure to identify what school your student attended prior to MHS and any sports he/she played at the varsity level at the previous school.

    InterScholastic Sports Examination Form

    *This is the ONLY medical form that the PSAL accepts. Any additional documentation provided by your doctor may supplement but not replace the InterScholastic Sports Examination Form.

    At the top of the form:

    • OSIS #
    • Student's name
    • Student's Address
    • Date of Birth
    • Emergency Phone Contact
    • Sport(s) your student intends to try out for during the school year.

    Additional Information:

    • Parental permission must be signed and dated.
    • Your doctor must stamp and date the form.
    • Doctors must also provide their registry number, address, telephone number, and signature for the form to be complete.
    • Doctors should also complete the student's medical history and physical examination sections of the form.
